When Do Testosterone Levels Decrease?

As men age, their testosterone levels decrease beginning in their early 30s by at least 1-2% each year. Low testosterone has many effects on men and although many treatments are available, including patches, mouth patches, gels rubbed on the upper body or thighs, oral medications, pellets inserted under the skin on the buttocks, self-administered intramuscular injections, and creams.

Is Testosterone Replacement Therapy The Right Choice For You?

Testosterone replacement therapy should only be considered by men who have had a blood test done confirming that is the cause of symptoms. Treatment is not recommended for men who do not have any symptoms or have low testosterone levels due to age.

Each treatment has its own advantages and disadvantages, but the decision is personal, and the patient should make their decision in conjunction with a doctor or men’s health specialist. Whatever method you choose, there has not been any research on the long-term risks and benefits of testosterone replacement therapy.

Testosterone Cream Options

Testosterone cream is prescribed under the brand Fortesta® and is available in a multi-dose pump.

Creams and gels have several advantages over other delivery methods. They also do not impact the liver or have other side effects that pill based medications have.

  • Less invasive- Testosterone cream is used in the privacy of your own home and does not involve the use of needles like injections or pellets. Long-term injections can lead to a buildup of scar tissue. No needles also means no pain.
  • More effective- With a slightly better increase in dihydrotestosterone (DHT) levels than other delivery methods, the cream provides therapeutic concentrations of testosterone and provides an eightfold increase in testosterone availability when applied to the scrotum without side effects, enhancing erectile strength and overall sexual functioning. This increase in DHT is crucial to a man’s libido and fluctuates daily, making daily use important for mimicking endogenous production of testosterone.
  • Faster absorption- because the medication does not have to be digested, the testosterone goes directly into the bloodstream providing immediate relief of symptoms.

The only downside unique to testosterone cream is the need to refrain from sex, working out, or showering for 90 minutes to make sure the product is absorbed completely. Accidental wiping off the cream through sex can cause hormone imbalances in the partner or birth defects in the unborn baby if the partner is a pregnant woman.

Testosterone Gel Options

Testosterone Replacement Therapy can also be administered through an alcohol-based gel that is usually applied to the chest, shoulders, and thighs. Because it is alcohol based, it should not be applied to the genitals. The gel also has a longer drying time of 2 hours before swimming, showering, dressing, or allowing anyone to touch you in those areas. 

Testosterone gel is sold under the brands AndroGel® (1% and 1.62% concentrations), Vogelxo™, and Axiron®. Like the cream, the gel is available in single-dose packaging and multi-use pumps. Vogelxo™ should not be applied to the stomach or abdomen. Axiron® should only be applied to the underarms at least 2 minutes after applying antiperspirant or deodorant with the metered dose bottle.

Proper Care Instructions

When using either the cream or the gel, wash your hands with soap and water before and after applying to the desired areas. Do not apply to broken or irritated skin. Care should be taken when priming pumps to rinse the initial product down the sink thoroughly and any messes during application are cleaned up immediately. When using single dose methods, throw away the packaging in a place where children or pregnant/nursing women cannot touch it.

Side effects of testosterone therapy in any form include:

  • Blood in the urine
  • Difficulty urinating
  • Frequent urination
  • Worsening sleep apnea
  • Acne or other skin reactions
  • Stimulating noncancerous growth of the prostate and growth of existing prostate cancer
  • Enlarging breasts
  • Limiting sperm production or causing testicles to shrink
  • Stimulating too much red blood cell production
  • Increased risk of heart disease
